Participants who did not complete a follow-up assessment had either moved out of the facility, moved to a higher level of care (eg, a nursing facility), declined further participation, or died.

Final diagnoses were generated through a local consensus procedure. Inclusion criteria were clinical stability (ie, no hospitalization or extended emergency department visit) for a minimum of 6 weeks and no significant (>20%) medication dose changes in the past 2 weeks. Participants with bipolar disorder had to have at least 1 mood episode recurrence or incomplete remission from a first episode, indicating stage 3 severity or higher according to the classification of Frank et al [41].

Scoring is a weighted accuracy score calculated according to the grid size shown. Faster response times are associated with the possibility of higher scores. The grid size is multiplied (eg, 4 × 4 = 16) and added to the running score of the previous correct trial. For example, for a correct trial with a grid size of 3×3 and a previous trial score of 246, the trial score would be calculated as follows: 3 × 3 = 9 and 9 + 246 = a trial score of 255.

To reduce the burden on sites and to increase the likelihood of consistent appropriate referrals, we held additional meetings with triage providers during in-services and created a pocket guide detailing the study inclusion or exclusion criteria and procedures.
Usual care in walk-in settings involves a diagnostic interview or intake with a triage provider (typically a social worker) and a psychiatric evaluation, which includes a standardized screening for suicidal thoughts and behavior.

A recent study by Checkroud et al [27] of over 7000 patients with major depression demonstrates why this approach, as well as any other that relies on indiscriminate use all of the items in a scale to assess primary efficacy outcomes (eg, the HAM-D), may be a problem. In their study, they illustrate how this indiscriminate approach to measurement can jeopardize a potential treatment in late-phase clinical trials.
