Preparing Patients and Clinicians for Open Notes in Mental Health: Qualitative Inquiry of International Experts

As one respondent noted: [M]any patients stumble over “patient complains of” or “affect” or other common psych mental health terms. One respondent suggested that it would be helpful for clinicians to be provided with: [a] list of words that appear to be judgmental or offensive that are frequently used (eg, patient lied, patient was aggressive, patient denied, patient is non-compliant, patient was upset). Participants also emphasized the need for training in writing notes with empathy and warmth.
