Video and Telephone Telehealth Use and Web-Based Patient Portal Activation Among Rural-Dwelling Patients: Retrospective Medical Record Review and Policy Implications

Telehealth modality was either video or telephone. Telephone visits were charted as “scheduled telephone” or “telephone” encounters; the latter are unscheduled billable calls from providers to patients. Analysis of unscheduled telephone encounters showed that they represented an important telehealth service (see Telehealth Modality by Rurality, Demographic, and Telehealth Encounter Characteristics section), and we therefore elected to retain them in our analysis.

The Finnish Strongest Families Smart Website (SFSW) intervention is an 11-week program that includes parent training delivered through a digital platform and assisted by weekly telephone coaching. Previously, we conducted an RCT, screening families at regular health checkups for 4-year-old children. Children who had parent-reported high levels of disruptive behavior were screened for targeted SFSW intervention.

As such, we administered the Mo CA [4] adapted for people with vision impairment (Mo CA-Blind [5]) via the telephone. We chose the Mo CA-Blind, given evidence that the telephone is a widely available device among older adults [6]. To supplement the capturing of this practice, we explored experiences and preferences related to general telephone use through a closed-ended questionnaire.

Most calls were made from a mobile telephone and it was not possible to determine the location from where these calls were made. Therefore, findings from this study may not be generalizable to all callers who contact crisis helplines as the analysis was limited to landline callers only. In addition, calls in Wales to the Welsh language line were not explored during this study.

In addition, we are proposing to deliver a GI intervention in a low-cost highly scalable way (ie, by telephone). To date, no studies have established the efficacy of an integrative GI tobacco cessation treatment delivered by telephone. Several meta-analytic reviews have shown that proactive telephone-based tobacco cessation services are an effective way to deliver treatment for tobacco dependence [13-15].

Several randomized trials compared the SF-36 scores issued from different administration modes, such as paper versus the internet [13-17] or telephone versus paper [18-26]. Telephone interview is a common mode of questionnaire administration for several reasons, including the potential to increase response rate [24-26], practical convenience if other data of the study are already being collected via telephone, and exploring Qo L in some special populations such as older patients.
