Chronic nociplastic pain includes conditions such as irritable bowel syndrome, interstitial cystitis, and the quintessential nociplastic pain condition, fibromyalgia, which alone affects more than 20 million Americans [1]. While the mechanisms underlying nociplastic pain are not completely understood, central nervous system–mediated amplification of pain and other sensory input, along with altered pain modulation, play a major role [2].

Tulgar and Senturk [10] and Tulgar et al [11] have described extensive and long-lasting pain relief after L-ESPB in hip arthroplasty. Conversely, Chan et al [12] found that L-ESPB did not reduce pain or opioid consumption in patients undergoing hip surgery. Given the lack of high-quality clinical trials evaluating L-ESPB in hip surgery, this study is warranted.

VR has demonstrated moderate benefits in acute pain management, including burns, labor, dressing changes, and periodontal procedures [15]. It is theorized that VR provides distraction from pain by multisensorial stimulation, perhaps via the gated-pain theory [7,14]. Reported side effects of VR are infrequent, mild, and self-limiting. They include nausea, vomiting, and eyestrain, collectively referred to as “cybersickness” [14].

Pain after total joint arthroplasty (TJA) is expected, but little is known about pain after discharge [1]. As the length of hospital stay after surgery continues to decrease, it is essential to develop our knowledge regarding pain and its management in the subacute postoperative phase. A recent meta-analysis found the prevalence of moderate to severe postoperative pain ranges from 51%, one day after discharge, to 58%, 1‐2 weeks after discharge [1].

Interstitial cystitis/bladder pain syndrome (IC/BPS) is a multifactorial, chronic urinary bladder syndrome. Common symptoms of IC/BPS include dysuria, dyspareunia, urgency, a persistent or recurrent sense of bladder pain or pressure that is often exacerbated as the bladder fills and is temporarily relieved after voiding, frequent urination or with little warning, nocturia that disrupts sleep, and associated burning or other abnormal urinary sensations [1].
