Secure Messaging for Diabetes Management: Content Analysis

Among the 121 patients who received a team-initiated thread related to diabetes, 88% (n=107) received a thread about medication renewals of refills; 85% (n= 103) about scheduling, referrals, or consults; and 73% (n=88) about medication issues (Table 1). Table 3 presents sample secure messages of each content type as found within the first message in a team-initiated thread.

Given the common recommendation for increased PA in patients with insomnia, these findings suggest that future research is needed to actively explore what impact, if any, PA can have for patients already using a common insomnia self-management program. In a recent pilot project, we used a combination of the CBT-i Coach app and a home-based objective sleep monitor to assess and provide feedback to post-9/11 veterans regarding their home-based sleep behaviors [40].

In the survey, participants were asked an open-ended survey question, “Can you tell us about an ‘A-Ha!’ Moment when you realized you could use the MHV portal to better manage your diabetes?” We selected interviewees to represent a variety of responses to this and other survey items about MHV use, including those who used a variety of MHV portal features, those with controlled and uncontrolled diabetes, urban and rural patients, and those with or without comorbid mental health diagnoses.

Patient engagement can operate at multiple levels and exists on a continuum [5]. We focus primarily on the level of direct patient care where engagement can range from patients receiving information about a diagnosis to patients participating in treatment plan decisions [5]. Although it has been characterized in a variety of ways, a common thread is that patient engagement is a multi-faceted concept incorporating actions on multiple levels to promote patient-centered care and self-management.

These sleep worksheets were embedded in a separate app that participants could also access from the i Pod Touch. Supplemental worksheets included Wakeful Activities, Coping Self-Statements, Constructive Worry, and a Relaxation Log. A self-management guide, in the form of a document accessible on the i Pod Touch, along with a paper copy, provided week-by-week suggestions for using elements of the app and the worksheets.
