Digital Conversational Agents for the Mental Health of Treatment-Seeking Youth: Scoping Review
A randomized controlled trial (RCT) of a digital conversational agent called Woebot found that, compared to a self-help e-book, the intervention significantly reduced depressive symptoms after 2 weeks, with a moderate effect size (d=0.44) [2]. In another RCT, 4 weeks of using Woebot was found to be noninferior to a standard clinician-led CBT group in reducing depression when comparing pre-post average scores and the number of participants with elevated depression; the effect size was not provided [30].

Participants who completed the demographics survey could be randomized to the TR-D condition, which showed participants a nontailored message (ie, brief description of PHQ-9 results) and featured resources tailored to age and LGBTQ+ status. A pool of 3 to 4 possible resources for each age group was chosen by an algorithm that identified the most popular resources within each age group.

These thresholds reflect the smallest clinically meaningful change, corresponding to a Cohen d of 0.33 based on prior studies' standard deviations [59,60]. Treatment fidelity was evaluated using a therapist self-report questionnaire administered 5 times throughout therapy, consisting of 6 Likert-style questions (ranging from 1 to 5) and questions about time spent on exposure therapy.
